ABSTRACT

Objective To investigate the clinical features and CT appearances of primary adenosquamous carcinoma (ASC)of the liver and to improve the understanding of this disease for the sake of misdiagnosis.Methods The clinical features and CT appearances in 7 patients proved by operation and pathology were reviewed,retrospectively.Results All of the patients had dull pain in upper abdo-men,fever,chills and a long history of cholangitis and biliary calculi.In addition,all patients had not the history of hepatitis B and liver cirrhosis.And the serum AFP level was normal.The single lesion was found in every patient including 4 lesions in the left lobe of liver and the other 3 in the right lobe.Plain CT showed all masses with hypodensity,heterogeneity and unclear edge in liver,and multiple irregular and more hypodense areas in lesions were found.Slight heterogeneous honeycomb-like enhancement in the arterial phase was showed.In the venous phase,persistent honeycomb-like enhancement with uneven separations,nodular bulge and hypo-dense necrotic area was found.In the delayed phase,further honeycomb-like enhancement with hypodense necrotic area and obvious-ly enhanced nodular bulge was showed.The bile ducts in the liver and around the mass were dilated and had pneumatosis in company with lithiasis in choledochus and intrahepatic bile duct in 5 patients.Conclusion Primary hepatic ASC has certain clinical character-istics in older patients.The CT characteristic features included:honeycomb-like lesions with persistent,heterogeneous,delayed en-hancement and heterogeneous separation,uneven inner edge and enhanced nodular bulge.

ABSTRACT

Adenosquamous cell carcinoma (Ad-SCC) of the colon is rare. The pathogenesis of Ad-SCC is unclear, however, several hypotheses have been suggested. The clinical presentation and gross findings of Ad-SCC of the colon are similar to those of adenocarcinoma of the colon, but Ad-SCC has a more aggressive clinical course and a poorer prognosis. We report on two cases of Ad-SCC of the colon with obstruction; a collision-type Ad-SCC that has not only obstruction but also numerous hepatic metastases, and a composite-type Ad-SCC treated with left hemicolectomy followed by an adjuvant chemotherapy.

ABSTRACT

PURPOSE: To define the clinical features and pattern of failure and to evaluate the results of radiation treatment in of adenosquamous cell carcinoma of the uterine cervix. MATERIALS AND METHODS: From Jun. 1981 to Dec. 1997, 43 patients with adenosquamous cell carcinoma of the uterine cervix were retrospectively analyzed external radiation treatment and HDR-ICR from Yonsei cancer center and Wonju cristian hospital. The median age was 51. Stage distribution according to FIGO were stage 1b in 10, 2a in 5, 2b in 18, 3b in 9, 4a in 1. Median follow-up period was 41 months. RESULTS: Overall survival rate and disease free survival rate were 57.2% and 60.2%. Complete response rate was 86.0%. Locoregional failure was observed in seven patients. CONCLUSION: Major pattern of failure was locoregional failure. Adenosquamous cell carcinoma was not more aggressive than other pathologic types.

ABSTRACT

Gastric squamous cell carcinoma is very rare and is termed both pure squamous cell carcinoma and adenosquamous cell carcinoma. The pathogenesis of this neoplasm remains obscure and theories vary widely. It tends to occur in the distal half of the stomach, frequently along the lesser curvature, and affects a slightly younger age group with a greater male preponderance compared to adenocarcinoma. One case of gastric squamous cell carcinoma and another case of gastric adenosquamous cell carcinoma is herein reported. In the first case, a 70-year old male was admitted as a result of epigastric pain. On endoscopy, abdominal CT and UGI, a huge fungating mass in the posterior wall of the antrum of the stomach was revealed. A subtotal gastrectomy with Billroth II anastomosis was performed. Unlike the endoscopic biopsy finding however, the histopathologic diagnosis determined a squamous cell carcinoma of the stomach. In the other case, a 64-year old male was admitted due to epigastric pain. Upon examination, a huge Borrmann type III tumor mass in the lower body and antrum of the stomach along the lesser curvature was seen. Endoscopic biopsy finding revealed a suspicious adenocarcinoma, and an operation was performed. Unlike the endoscopic biopsy finding however, the histopathologic diagnosis revealed a adenosquamous cell carcinoma of the stomach.

ABSTRACT

Primary adenosquamous carcinoma of the gallbladder is an extremely uncommon neoplasm which accounts for approximately 2% of carcinomas of the gallbladder. According to a recent extensive review of carcinomas of the gallbladder, the dismal outlook for this carcinoma still prevails in spite of recent advances in diagnostic and surgical techniques because of late diagnosis and ineffectual treatment. A sixty-one-year-old female patient visited the Department of Internal Medicine due to postprandial epigastric discomfort. Abdominal sonography, abdominal computed tomography (CT), and endoscopic retrograde cholangiopancreatography (ERCP) were performed. We treated the patient with a cholecystectomy and chemotherapy with FAM. However, five months after discharge, the patient revisited, complaining of epigastric discomfort. We performed abdominal CT and gastrofiberscopy. The result was a recurrence of the cancer in the bed of the gallbladder and in the liver. However, the patient and her family did not want more treatment for the metastase of the cancer. We experienced one case of an adenosquamous-cell carcinoma of the gallbladder, and we report this rare cancer with a review of the literature.
